    Shannon Taush and Matt Morony recieved the 2017 Plaza Healthcare Employees of the Year Award. The banquet which was held on February 22, 2018 included twelve 2017 Employee of the Month winners and their guests. This year's banquet was inique in that we were not able to choose just one winner. We awarded 2 amazing employees the award.

  &lt;p&gt;&#xD;
    
                    
    Shannon Tausch has been with Plaza in the Case Management Department for 3 years. She is originally from San Jose, California. She became interested in Healthcare as she in her own words would say "I'm accident prone and I spent a lot of time in medical facilities". When Shannon is not working she enjoys hiking, traveling, and cooking. She calls herself "Chef Shanny".

  &lt;p&gt;&#xD;
    
                    
    Matt Marony has been with Plaza Healthcare for 6 years. He is originally from Oregon and relocated to Arizona 17 years ago. He is one of our very best Licensed Practical Nurses and actually trains the majority of our new nurses. He became interested in Healthcare after being in the Air Force for 6 years. When Matt is not working he enjoys hiking, reading, and flying airplanes with his father.

    Zach Richardson received the 2016 Plaza Healthcare Employee of the Year Award. The banquet, which was held on February 16, 2017 included twelve 2016 Employee of the Month winners and their guests. Only one employee out of 260 eligible employees receives this prestigious award annually.
  
                  &#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    
                    
    Zach has been a member of Plaza’s nursing team for ten years, and has been an LPN for 15 years. Before becoming a nurse, Zach was a professional musician, He made a life-changing decision to become a nurse because he enjoys helping others.
  
                  &#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    
                    
    Zach is originally from Ottumwa, Iowa and moved to Arizona in 2000 for warmer weather. He has an eight year old daughter and twin daughters aged 5. When not at work, Zach enjoys spending time with his kids swimming and hanging out. He also enjoys playing music with his dad.

  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;

    &lt;!--StartFragment--&gt;                          On August 12, 2016, Plaza Healthcare Staff celebrated their Deficiency Free, Annual DHS Survey. The day started with a food truck handing out free waffles to the night shift and incoming day staff. Festivities continued throughout the day with another food truck providing lunch for the entire building, giveaways of Beats headphones, IPads, and a 55" Television and the ever popular money cube. All staff had 20 seconds to grab as much cash and prizes as they could. Some staff grabbed vacation time, free lunches, fitness watches, and $100 bills. Overall it was a great day, for a tremendous staff, who deserved to be appreciated for an amazing job not just during the survey, but throughout the year. Congratulations.

     by the American Health Care Association and National Center for Assisted Living (AHCA/NCAL). The award is the second of three distinctions possible through the AHCA/NCAL National Quality Award Program, which recognizes providers across the nation that have demonstrated their commitment to improving quality of care for residents and patients in long term and post-acute care centers and communities.

    “This award is a symbol of our team’s commitment to the pursuit of quality excellence,” said Barb Crivellaro, Chief Clinical Office of Plaza Healthcare. “Every day, 24/7, the women and men of Plaza Healthcare work to ensure the highest level of quality and person-centered care to our residents, patients and their families. Plaza Healthcare is the first facility in over 20 years to achieve this award from the State of Arizona.

            Arizona Health Care Association (AHCA)

      &lt;/u&gt;&#xD;

          , the state’s largest association and advocacy organization for skilled nursing and assisted living communities. The honor was presented in June at AHCA’s annual statewide conference in Scottsdale, Arizona.

          The award is one of seven statewide honors recognizing individuals and organizations that go above and beyond to provide exceptional care to Arizona’s elderly and disabled residents.

          Tammy Evory serves as the Assisted Living Manager at Mimosa Springs. Under her leadership, the staff created new amenities for residents including a spa program and sensory stimulation room and tripled the activity program to include live entertainment weekly, cooking classes, games, and family gatherings. Staff members report that rather than giving orders, Tammy leads in a collaborative way, inspiring them to share her vision and believe in themselves.
         &#xD;
  &lt;/p&gt;&#xD;
  &lt;br/&gt;&#xD;
  &lt;p&gt;&#xD;
    
          “These outstanding professionals have chosen to work in a career that is complex, challenging and also deeply rewarding,” said AHCA Executive Director Kathleen Collins Pagels. “Each of the caregivers and organizations selected for these awards – along with all those who were nominated – represent the best-of-the best of our profession.”

    Plaza Healthcare celebrated Lucy Pillman’s 30 year work anniversary on February 26, 2015. The entire day was devoted to Lucy. There was a staff and resident barbeque and a celebration at the facility’s All Staff Meeting where she received a crystal trophy. There was also a large banner hanging in the lobby in her honor.

    Lucy, originally from Illinois, moved to Arizona with her husband in 1984 to seek better job opportunities. At the time, Plaza Healthcare was called Scottsdale Community Hospital. Her husband John first was hired by Scottsdale Community Hospital and later Lucy joined him by taking a job in the Dietary Department.
  
                  &#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    
                    
    During her thirty years, Lucy has made many friendships with the staff and residents who have become like part of her family. She has watched employees grow and evolve just like her own children and feels a connection with all of them. According to Lucy, “the family environment of Plaza is what has kept me working here for so long.” She has always felt like she is treated with the upmost respect from everyone at Plaza and enjoys coming to work each day. When she leaves for the day she always has a smile on her face and looks forward to her next day at Plaza. Seeing the residents each day makes her job enjoyable and is ultimately her favorite part of her job.
  
                  &#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    
                    
    Lucy was married for 48 years and has two children, four grandchildren, and five greatgrandchildren. According to Lucy, she has no plans of retiring anytime soon and looks forward to another thirty years at Plaza. When she isn’t working, Lucy loves to spend time outdoors doing yardwork and enjoys her time with her family.

    zabela Sawicka received the 2015 Plaza Healthcare Employee of the Year Award. The banquet, which was held on February 25, 2016 included twelve 2015 Employee of the Month winners and their guests. Only one employee out of 260 eligible employees receives this prestigious award annually.

  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    
                    
    Izabela has been a member of Plaza’s nursing team for over two years. According to Izabela, she received her inspiration to become a nurse from her mom who is also a nurse. “Seeing the patient’s improvement and watching them leave the facility in better shape than when they arrived is the most rewarding part” according to Izabela.

  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    
                    
    Izabela is originally from Poland but was raised in New Jersey. She moved to Arizona to be closer to family. She is engaged to be married and has one dog. When not at work Izabela enjoys reading and traveling.

    For the second consecutive year, Plaza Healthcare has participated in the Arizona Special Olympics’ Over the Edge event. On December 12, 2015, eleven Plaza Employees rappelled down City Scape, a twenty-seven story glass building in downtown Phoenix.
  
                  &#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    
                    
    The facility set a goal in April of 2015 to raise $11,000 and through the help of our community, staff, friends, and family, Plaza was able to raise $11,250. According to the Special Olympic Committee, Plaza finished 4th in the state of Arizona for monies donated. Overall, the Over the Edge event raised more than $210,000 ensuring that over 400 athletes would receive a full year sponsorship.

    On August 20, 2015, in front of thousands at the Arizona Health Care Association Awards Banquet located at the Talking Stick Resort in Scottsdale, Norm Doty received the Resident of the Year Award. This award was a surprise to Mr. Doty. His sons Stephen and Tim were also in attendance when Mr. Doty received his award.

  &lt;p&gt;&#xD;
    
                    
    Gayle Borter, the Director of Activities submitted the Resident of the Year nomination. According to Gayle, “Norm’s calm and caring nature lends stability and a welcoming atmosphere in our activity program. He meets and greets new residents and encourages involvement in the activity program. He greets and welcomes residents, staff and vendors. He is known to greet our older residents with “Hello young lady, how about a movie tonight?” He watches over his neighbors during Bingo ensure confident wins.”

    &lt;b&gt;&#xD;
      &lt;br/&gt;&#xD;
    &lt;/b&gt;&#xD;
    
                    
    Mr. Doty grew up on a dairy farm in Prattsburgh, NY. As an adult he drove truck for 38 years. He was active with his family and his faith. He has been a resident at Plaza Healthcare since June of 2013.
  
                  &#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    
                    
    Organized in 1953, the Arizona Health Care Association has grown into a strong and viable group of long term care providers dedicated to enhancing the quality of life for Arizona’s elderly and disabled in residential settings. Today, membership encompasses the majority of licensed, operating nursing care facilities in the state, who together provide continuous nursing care and assisted living services to over 15,000 residents. AHCA was founded to promote standards and ethics in the profession of long term health care, and to ensure quality care for nursing facility and assisted living residents. AHCA is a non-profit organization supported by membership dues, educational program fees and other services.

    Plaza Healthcare has been recognized with a 2015

      Bronze - Commitment to Quality Award

    by the American Health Care Association and National Center for Assisted Living (AHCA/NCAL). Plaza is one of two skilled nursing facilities to receive this award in the State of Arizona. The award is the first of three distinctions possible through the AHCA/NCAL National Quality Award Program, which honors long term and post-acute care centers that have demonstrated their commitment to improving quality care for seniors and individuals with disabilities.

  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    
                    
    “It’s gratifying to see these providers recognized for their commitment to improving quality care,” said AHCA/NCAL National Quality Award Board of Overseers Chair Christine Boldt. “At this level, Bronze recipients are positioned to achieve even better health and organizational outcomes. I applaud and encourage this year’s recipients to continue their quality journeys.”

    Implemented by AHCA/NCAL in 1996, the National Quality Award Program is centered on the core values and criteria of the 
    
                    &#xD;
    &lt;i&gt;&#xD;
      
                      
      Baldrige Performance Excellence Program. 
    
                    &#xD;
    &lt;/i&gt;&#xD;
    
                    
    The program assists providers of long term and post-acute care services in achieving their performance excellence goals.
  
                  &#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    
                    
    The program has three levels: Bronze, Silver, and Gold. Centers begin the quality improvement process at the Bronze level, where they develop an organizational profile with fundamental performance elements such as vision and mission statements and an assessment of customers’ expectations. Bronze applicants must also demonstrate their ability to implement a performance improvement system. Trained Examiners review each Bronze application to determine if the center has met the demands of the criteria. Centers may now move forward in developing approaches and achieving performance levels that meet the criteria required for the
